That was me, Tiger.

Ball landed on my eyes, broke the frame of glasses and that caused a little cut.

Subject: FW: Incident Report from (8/27 through 9/2/06)

Hi guys,

Noticed the routine bulletins about accidents included a soccer player. Anyone know who this was and what happened?

See the forwarded message below.

Greetings:

During the previous week, we had two reported injuries. One incident was not work related.

The FY06 cumulative total will be updated on the Real Time Injury Page when ES&H receives more information. https://www-internal.slac.stanford.edu/esh/general/accidents/resources/realtimecase.htm

Here are the details:

1)	An employee was struck in the eyelid while playing soccer and suffered a cut to the eyelid. Not work related.

2)	An employee reported an sprain/strain to his/her index finger. First Aid only. The incident is still being investigated.

Also, please see the newly formatted lessons learned page http://www-group.slac.stanford.edu/esh/concerns/lessons_learned/

Regards,

Linda Knudsen
