Presumably this occurs in Linux but what Linux? It would be helpful to get a core file and an lsof of the server when this occurs. The key problem is the exclude error that should have never happened. The loop is inevitable when this occurs (a feature of epoll). Hopefully, this doesn't happen too often but when it does, please provide the two pieces of information so we can track down the issue.
