Hi all,
  Thanks for all the information provided, and for pointing towards the issue. 
  I found that this only appeared to affect async transfers; the size of the overrun was related to half the async segsize, and the issue only seemed to appear for client request sizes up to half the segsize value. 

We're testing a fix: https://github.com/snafus/xrootd-ceph/commit/dd49fff147276bc6f2b0ca6795454c8922c2d070 
which appears to work against the code snippet from Michal, and against the original copy command at the top of this issue. 
Assuming that this is the solution (and no unintended consequences), I'll hope to make the various pull requests.
